SouthCountyConnects.org is a social networking website dedicated to building social capital and increasing civic engagement in the communities served by the CHNA 5. This community building tool was initiated by community partners as a launching pad to develop a site to connect people to people; people to resources & opportunities, people to groups & organizations; and people to ways to get involved in their neighborhood.

One of the main focuses for Community Partnerships this year
is the development and implementation of community outreach
to families with children ages birth though five years of age
who may be at risk, or hard to reach. An organizational meeting
was held last week for community agencies that may be able to
help with this project. One outcome of the meeting was the decision
