PARENT SURVEY

Dear Community Partnerships Council Member:
One of the main focuses for Community Partnerships this year
is the development and implementation of community outreach
to families with children ages birth though five years of age
who may be at risk, or hard to reach. An organizational meeting
was held last week for community agencies that may be able to
help with this project. One outcome of the meeting was the decision
that the schedule, locations, and content of this outreach must
meet the needs of the families on our towns.
Attached is a parent survey that I am asking you to print, copy,
and ask all families with young children to complete. Also attached
is a registration form for parents to complete to be entered into a
drawing at the end of the survey time. For those of you with a web
site, it would be greatly appreciated if you would post the link below,
so those families with internet access can complete the survey on line.
They can also complete the registration form (Unfortunately this is
available in hard copy only).
The survey will end on September 30. I will contact you at that time to
collect completed surveys and registration forms. I understand the difficulty
in getting parents to fill out “one more piece of paper”, but the success of
this outreach depends on how we are able to meet the needs of the community.
